CCRC Physical Therapy participates in most insurance networks.  Please bring all insurance cards to your first appointment so that we may properly store all of your insurance information in our system.  If you are covered by Tricare then your valid Military Id will be requested.

Please be prepared to pay your co-pays as services are rendered.  Our staff understands that illnesses and injuries often cause financial stress and we will make all efforts in order to work with your situation.  Please feel free to speak with the Office Manager at the location where your care is being received.

• Medicaid (birth to age 21)
• Tricare Standard & Tricare for Life
• Tricare Prime (requires referral from Primary Care Provider)
         

We also accept Workers Compensation Insurance.  Several of the companies we accept are Medrisk, Liberty Mutual, Nationwide, Network Synergy, Keyrisk, Travelers, and Zurich North American.

Our staff will verify your Physical Therapy benefits as provided by your insurance carrier.  If you have questions in reference to your insurance we will be happy to assist you or you may also contact your insurance carrier in regard to your benefit.

Please keep in mind that the insurance companies listed above are some of the more common ones we file.  If you do not see your carrier listed please contact our office and we will gladly assist you.
